Every year, hundreds of forms are created by Graphics for University, Hospital, Health System, and Jefferson University Physicians’ use. We offer professional typesetting and layout, client consultation, Medical Records and Forms Management Committee approval assistance, and Printing Center coordination. All our forms are digitally archived for easy future revision. Up-to-date with the latest Hospital and University formats, including barcode requirements and current corporate logos, Graphics provides complete forms design service.

So, what is a well-designed form? It is unambiguous and well organized. Good typography and composition lead the user from top to bottom, through an orderly sequence. It has clear, easily understood instructions and enough space to accommodate the users’ entries. Most importantly, a well-designed form fulfills its function as an instrument for the collection and distribution of information. It meets your needs and provides a consistent professional appearance to both patients and staff.
